To Parents of Elementary Age Children:
RE: Follow-up 2nd dose for the nasal formulation (LAIV) of vaccine against Influenza A (H1N1)
On Friday, January 22, 2010 the Dickinson County Health Department will return to Herington Elementary School to administer the 2nd dose of LAIV – Live Attenuated Influenza Vaccine to those students 5-9 years of age who received the first dose on December 18th. They will allow any child ages 5 -9 years of age to come for the first dose as well. The consent form needs to be completed (front side only) and returned to school before January 22nd if you wish your child to be vaccinated in the school based clinic. The consent forms will be reviewed for completeness as well as to verify that there is no contraindication to the child receiving the LAIV vaccine.
It is imperative that if your child has received one dose already, please indicate this on the consent form. If your child has received any of the following vaccines recently, please indicate that on the consent form as well: MMR (measles, mumps, rubella); Varicella(chickenpox); Rotavirus.
Chronic Medical Condition: If your child has a chronic medical condition as defined below, we ask that the parent accompany them to a walk-in clinic to be vaccinated as the child is not eligible for the LAIV.
Chronic medical conditions are defined as chronic lung(including asthma), heart, liver, kidney, cognitive(including Down’s syndrome), neurologic(seizures, epilepsy), blood or metabolic disorders(including diabetes) or a condition that suppresses the immune system. Children who have been diagnosed with any of the above, will not be vaccinated at school, rather parents are encouraged to bring them to a walk-in clinic to receive vaccination.
Parental consent is required for any child to receive the vaccination. The form is attached.
