All students enrolling in any
district school shall provide the building principal with proof of
immunizations of certain diseases or furnish documents to satisfy statutory requirements.
Booster shots required by the Secretary of the Department of Health
and Environment are also
required.
A copy of this policy and the
applicable state law shall be distributed to students or their
parents each fall and spring. The superintendent shall issue a news release each August
explaining the required inoculations and booster shots.
Parents may delegate in
writing their authority to consent to immunizations. If the parent
is not reasonably available, and the authority to consent has not been denied as provided
in law, individuals other than the parent may consent to the
immunizations as provided
for in current law.
Students who fail to provide
the documentation required by law may be excluded from school by
the superintendent until statutory requirements are satisfied. Notice of exclusion
shall be given to the parents/guardians as prescribed by
law.
Each principal shall forward
evidence of compliance with the inoculation law to other schools or
school districts when requested by the school or by the student's
parents/guardians.
To comply with the policy of
this school district, students entering school for the first time
must have received:
- four (4) doses of DPT/DTaP, if the 4th
dose was received after the 4th birthday or five (5) doses of
DPT/DTaP if the 4th dose of DPT/DTaP was received before the 4th
birthday,
- three (3) doses of OPV/IPV if the 3rd
dose of OPV/IPV was received after the 4th birthday or four (4)
doses of OPV/IPV, if the 3rd dose was
received before the 4th birthday
- two (2) doses of combined Measles, Mumps,
Rubella (MMR)
Students are required by the
Kansas State Department of Health to have a booster dose of TD
vaccine 10 years after completing the primary vaccination series for school
attendance.
On or before May 15 of each
school year, the school board shall notify the parents or guardians
of all known pupils who are enrolled or who will be enrolling in the school of the law
and school policy regarding the immunization policy.
If a pupil transfers from
another school, the school from which the pupil transferred will be
asked to forward with the pupils transcript the certification or statement showing evidence
of compliance with the requirements of the law.
If a student has not been
notified prior to enrollment of the requirements or the school has
not received documentation from another school showing evidence of compliance with
requirements the student will have 60 days to be in compliance or
the exclusion policy will
be followed.
As an alternative to the
certification required the pupil shall present:
1. A written statement signed
by one parent or guardian to the building principal that they are
an adherent of a religious denomination whose religious teachings are opposed to such
tests of inoculations.
2. An annual written
statement signed by a physician stating the tests of inoculations
would endanger the life or health of the child.
Students who fail to provide
said documentation may be excluded from school by the
superintendent or his designated representative until the aforementioned requirements are
satisfied. Notice of exclusion of attendance shall be given to
the parents/guardians as
prescribed by law.
Parents will be given written
notice indicating the reasons for exclusion from school attendance;
the notice shall state that the student will continue to be excluded until the provisions
of law are met; and also to inform the parents/guardians that a
hearing will be provided
upon request by the parents/guardians.
A copy of this policy shall
accompany applicable state law which shall be distributed to all
students at the beginning of the school year. The superintendent shall annually issue a news
release regarding this requirement of state law and local
district policy.
The appropriate principal
shall forward evidence of compliance with health tests and the
inoculation law to other schools when requested by such schools or a child's
parents/guardians.
